Regular reviews, clear targets.
Follow-up consultations are available once a week. Some conditions also need lab tests, in-person checks or a specialist, and your doctor will tell you when.
Diabetes
Share your sugar readings and recent HbA1c report. The doctor reviews your medicines, diet and activity, and sets targets that suit you.

Home BP readings help most. Measure twice in the morning and twice in the evening for a week before your consultation. If a reading is above 180/120 with chest pain, breathlessness or a severe headache, call 112.
Thyroid conditions
Bring your latest TSH and thyroid reports and a list of symptoms. The doctor reviews your dose and how often to repeat tests.
Cholesterol
A lipid profile report lets the doctor look at your overall heart risk and advise on diet, activity and medicines.
Anaemia and vitamin deficiencies
Haemoglobin, vitamin B12 and vitamin D reports help explain fatigue and weakness. The doctor advises on treatment and when to recheck.
